Documents required for ‘Exit Permit’ :

A) In case of foreigners reporting due to loss of passport 

a) New passport or Emergency Travel document issued by the concerned foreign Mission.
b) Police reporting about the loss/stolen passport i.e. loss certificate or F.I.R.
c) Consulate’s letter indicating the particulars of stolen passport and Indian visa.
d) If the foreigner is a registered with any FRRO/FRO then a NOC to that effect from the concerned FRRO/FRO.
e) Confirmed Air Ticket for departure with OK status for returning form CSI Airport Mumbai.
f) Four passport size photographs (4cm × 4cm with clear background ears clearly visible, no spectacles and caps.
g) Proof of Residence i.e. Hotel / Guest House Payment receipt, Leave & Licence agreement etc.
h) All cases of ‘Exit Permit’ would require minimum 2 working days for processing.
i) Apart from the above documents additional supportive documents can be asked for in certain cases.

B) Surrogacy cases –

a) Concerned Consulates/Missions letter.
b) Surrogacy agreement between the applicant surrogate mother & doctor treating the case.
c) Doctor’s letter who has given treatment.
d) Letter from hospital where infant was born.
e) Infant’s (Applicant) passport.
f) Infant’s birth certificate issued by Indian authority.
g) Parent’s passport and visa copy.
h) Air Ticket (Infant + parent’s)
i) Infant’s photo – 3 (with white background)j)
j)'No objection letter' from Surrogate mother.
k)"No dues are pending" letter from hospital

It would require 1 to 15 working days for processing case of  'Exit Permit' in respect of Surrogate Child depending upon the place of surrogacy aggrement, place of  birth of child etc. 

C) Child born in India -

a) Letter from Consulate regarding issuance of new passport.
b) Birth Certificate issued by Indian Authority
c) Both parents passport and visa copy, Residential Permit copies (in case parents are registered)
d) Air ticket (Infants + Parents)

In case of infant / minor children, travelling with mother only, N.O.C. from father, stating that he has No Objection for child/infant travelling abroad and vice versa.

D) In case of release from Jail / acquittal in case -

(i) Original passport / Emergency Certificate or Travel document.
(ii) Letter from Consulate regarding issuance of new Passport / Travel Document / Emergency Certificate.
(iii) 4 passport size photographs (35 mm × 35 mm with clear background ears clearly visible, no spectacles and caps.) 
(iv) Jail release Certificate / Courts order in case of acquittal.
(v) ‘N.O.C.’ (No Objection Certificate) from prosecuting agency that they would not be going in appeal and have no objection to the concerned foreigner leaving the country, in acquittal cases.
(vi) Confirmed Air Ticket for departure from C.S.I. Airport, Mumbai with OK status.
